Services:
Pathways is a special care unit within Three Links Care Center and serves those residents who are experiencing the later stages of Alzheimer’s disease or other related dementias. The philosophy of Pathways is to provide an environment that is sensitive to the needs of residents who exhibit symptoms or wandering or behaviors that may be disruptive to others. The staff works together to provide care that will maximize the residents’ strengths and maintain his or her level of independence for as long as possible. Pathways believes that residents should be free from physical and chemical restraints to promote the freedom of wandering. The nursing staff works closely with the physician, nurse practitioner, consultant pharmacist and the Three Links Medical Director to adjust medication usage for each resident. Finally, though the environment has changed, Pathways believes that the people who played a significant part in a resident’s life should be actively involved in his or her care. Family members are encouraged to become partners with staff in the care of their family member. This self-contained, community-within-a-community, provides a secure and tranquil environment for those facing the challenges of dementia or memory loss. Trained caregivers provide personalized care and supervision 24 hours a day. Services include:
