WELCOME TO THE THREE LINKS CARE CENTER – The Care Center at Three Links offers long-term healthcare in a comfortable setting to achieve optimum independence. A dedicated nursing staff carry out a plan of care for each resident and Nursing Assistants assist with personal care needs. People tell us when they enter Three Links Care Center they can feel a difference here – an atmosphere that is warm and welcoming. We have a beautiful campus that people can call home, however, it is our staff that truly makes Three Links a special caring place. Those who work here, no matter their position, complete their care duties with a passion in their heart. They want to do the best work they can do to care for each person with dignity and respect.

If Three Links becomes a temporary or permanent home, we believe we need to accompany each individual, as well as family members and friends on the journey of older adult services. It is our desire to make each person’s stay as comfortable and meaningful as possible. It is our intent to offer support and education to further the family’s understanding of our options for care. Caregiver Support Group

Finding social support is one of the best ways caregivers can cope better over time. Caregivers become absorbed in daily routines and forget that others face similar struggles. It can be a relief to hear your fellow caregivers’ experiences and learn that you are not alone. The meetings are facilitated by Three Links’ Social Workers and tailored to your interests, concerns and questions. This group is open to the Northfield community for family and friends who are caregivers of a loved one with any form of dementia.

Join us the 2nd Wednesday of each month from 4:00pm – 5:00pm at Three Links Care Center, 815 Forest Avenue, Northfield, MN 55057

Family Council Meetings at Three Links Care Center

 Family Council the 3rd Tuesday of every month from 4:00pm – 5:00pm in the Chapel Conference Room at the Care Center. Due to the holiday schedule, the next meeting will be Tuesday, January 16th.

Family Council is a group composed of residents, family and friends of residents of Three Links Care Center and Pathways Memory Care. Family Council meetings provide orientation, support and information for new residents and families. The meetings allow for family input into decisions and changes taking place as well as a  means for processing concerns and solving problems. Lisa McNeil, RN DON – Three Links Care Center Director of Nursing

 

Lisa McNeil, Director of  Nursing – Lisa McNeil has joined the team at Three Links as Director of Nurses.  In her role, Lisa will oversee a staff of 150 care providers including Registered Nurses (RN), Licensed Practical Nurses (LPN) and Certified Nursing Assistants (CNA), as well as other staff which support the nursing department.  It will be imperative for Lisa to assist campus leadership in creating a culture where all team members can be successful.  As Director of Nurses, Lisa will plan, organize, direct and evaluate the nursing department in order to provide quality care in accordance with the philosophy and policies of the governing body as well as the regulations established by state and federal laws.

As a RN, Lisa has a Bachelor of Science from Bemidji State University in Bemidji, MN.  Lisa has worked in all roles including as a CNA, a LPN, as well as a RN.  Through her nursing experience, Lisa has served in various roles including Director of Nurses, Supervisor, Case Manager, and Clinical Manager.  Her “floor experience” has included electrocardiogram care, pulmonary care, IV care, wound care and the use and education of wound VACs, medication and injection administration, blood draws, mental health, clinical nursing, as well as, direct care. In one role, Lisa worked as a Hospice RN where she worked in care facilities and people’s home providing case management.

Originally from the Duluth area, Lisa and her husband Chris live in the south metro with hopes to relocate closer to the Northfield area. Together the couple have five children; four boys and one girl.  The family enjoys camping and fishing as well as sporting events. 


Transitional Care at the Care Center: The Bridge

 

The Bridge at Three Links provides transitional care during the recovery period from injury, surgery or other neurological impairments. The goal is for each resident to achieve the highest level of independence through individualized treatment plans thus ensuring a successful return home.. A team of highly qualified and caring rehabilitation professional address issues of mobility, performance of daily activities and communication skills to restore lost abilities that affect return to independent life. Our excellent rehabilitation services are provided by RehabCare.


Meet Three Links’ Chaplain, Clark Cary

 

I’m Pastor Clark Cary and I am currently the Chaplain at the Three Links Campus. I’m a Pastor of the Evangelical Lutheran Church in America and my wife, Lois, and I have lived in Northfield for the past 13 years. I served congregations in Pasadena, California and San Francisco, California before e moving to Blue Earth , MN where I served a congregation for 22 years. Since moving to Northfield, I’ve served five different congregations over a period of ten years as an interim Pastor. As a parish pastor there have always been opportunities to  serve people of all ages and I’ve enjoyed all aspects of ministry. My enjoyment of being a pastor and working with older member of the church has always been an important part of my ministry. Three Links is an excellent setting for continuing active ministry. Although this is a part-time ministry I take seriously the opportunities to learn to know and care for residents and staff here at Three Links. The care and nurture of residents and their well-being is uppermost in my mind. I look forward to serving you as a pastor and friend. – Pastor Clark Cary
