Our mission is to provide care options that encourage the people we serve to function at their maximum level of independence in a respectful, clean and secure environment. We are totally committed to resident care as the staff responds to the ever-changing care needs of the people Three Links serves by developing timely, innovative programs and services.

Our community includes: a 102-bed Skilled Nursing Care Center with a secure special-care unit for those with advanced Alzheimer’s disease and other dementia; The Cottages providing supervised living in a home-like environment for residents with early memory loss; Cottage on Forest, specialized memory care; Millstream Commons, a 44-unit assisted living facility; Reflections~Care Suites with hospice services for end-of-life care; Three Links Apartments for independent senior living as well as adult daycare and home care services.
