Mission Statement
The mission of Three Links Care Center is to provide care options that encourage the people we serve to function at their maximum level of independence in a respectful, clean and safe environment.
Three Links Care Center is a 102-bed skilled nursing care facility. Twenty-four beds are dedicated to the Pathways Unit, which is a secure unit for those with Alzheimer’s disease or other dementia. A Transitional Care Unit provides skilled care and rehabilitation services during the recovery period from injury, surgery or other neurological impairments.

Services
- Licensed nursing staff on duty 24-hours a day
- Social services available to assist residents and families
- Pastoral care through church services and individual time
- Food service under the direction of a registered dietician provides healthful and attractive meals
- Housekeeping and laundry
- Activities designed to assist residents in adjusting to their environment in a meaningful way
- Physician’s services – prior to admission, each resident must designate a physician who is responsible for their care. Three Links Care Center’s medical director is responsible for reviewing resident care policies.
- Optional services (at an additional cost) include: laboratory, optometrist, dentist, pharmacy, physical, occupational and speech therapy, and beauty shop.
